We are looking for a Patient Navigator to play an important role at AndHealth. In this role, you’ll help guide the patients through their journey with AndHealth with the ultimate goal of creating incredible health outcomes and ensuring they receive the care needed to live a healthier life. You’ll work across the Care team with our Physicians, Nurses, Health Coaches, and Patient Services team, as well as provide feedback to our product team on how to best deliver a "wow" experience.

What you’ll do in the role:

  • Build trusted relationships with patients through different modes of communication (video, calls, chat messages, and sometimes in-person interactions)
  • Teach patients how to engage with different parts of their care, like the AndHealth app, telehealth visits, or our mobile labs service
  • Maintain consistent engagement with patients to ensure each patients know what their next steps are and nothing is falling through the cracks
  • Coordinate care by scheduling visits, helping patients access medications, and making sure lab work or other logistics are completed
  • Answer patient questions and resolve issues quickly, clearly, and kindly — even when things are complex
  • Collaborate with health coaches, pharmacists, and providers to make sure patients feel supported and seen every step of the way
  • Go the extra mile to create “wow” moments that make patients feel genuinely cared for
  • Help us improve the way we work by flagging gaps or inconsistencies, documenting new processes, and being an active participant in our daily troubleshooting and brainstorming session.

Education & Licensure Requirements:

  • Bachelor’s degree is preferred.

Other Skills or Qualifications:

  • Experience working in a clinical setting (ideally outpatient).
  • Passion for bringing the AndHealth programs to those who need it most.
  • You strive to “wow” in each interaction, both with patients and your colleagues.
  • A strong sense of ownership and ability to take initiative to make each patient’s experience better than the last.
  • A builder’s mentality and willingness to create a process where needed along with follow-through on those processes.
  • Ability to thrive in change and quickly adapt to evolving processes to help improve the patient and care team experience continuously.
  • Excellent written and oral communication skills.
  • Strong attention to detail and ability to successfully handle multiple things at once.
  • You bring a servant-leadership approach to your interaction with everyone.

  • You are comfortable with technology and learning new systems.
